    Just to clarify, I told them that I didn't have the form with me but asked if they wanted to see it. I was told that they didn't want to see it when I went next time, as I would have to reclaim the money myself each time I go.

    Changing practice again is hardly an option, there are so few-a NHS dentists these days!

    it'll be on the internet, just have a search. but you usually get a big envelope pre paid WITH the claim form anyway

    Could someone please provide me with relevant documentation that states that the Dental Practice should be responsible for making these claims when they are shown a valid HC2 form? If anyone could, I would be extremely grateful (I'll take it with me next time I go so that I am not put through so much inconvenience in future).
