    Monday, 17 March 2008 at 11:59 p.m

    Eligibility:
    You may nominate someone to participate in the Mouse Trap game (the “game”), on the Ant & Dec’s Saturday Night Takeaway programme (the “programme”). The person to be nominated, (the “nominee”), must be available on the relevant date, and must agree to take part in any filming, broadcast and publicity related to the programme. It is the responsibility of the person nominating someone to ensure that the nominee is available on the date and time as specified.

    The nominee must be a resident of the United Kingdom aged 18 years or over. Employees and members of their immediate families (including any live-in partner) of companies within the ITV group of companies and any of their subsidiaries and/or agencies associated with the game on the programme, including the producer of the programme and/or the game, are ineligible to nominated. Any such nominations will be invalid. The nominee must be physically fit, with no known injuries or medical condition or pregnancy. It is the responsibility of the person nominating to ensure that information provided concerning the nominee is accurate and up-to-date. Providing information which is inaccurate or not up-to-date will mean that your nomination is invalid and the nominee may not participate in the game on the programme. The Promoter will not be held liable in the event that such information is inaccurate or not up-to-date in any way.
    The nominee must meet the eligibility requirements to participate in the game on the programme.

    Nominating a person to participate in the programme and/or the game signifies acceptance of these terms and conditions.

    Method of nomination:
    You may nominate someone to participate in the game on the programme by going online to the website at www.itv.com/takeaway and completing the online entry form as instructed. It is not possible to nominate someone via the post or via an email.

    You are required to provide the following information on the online nomination form, including but not limited to:

    ·Your full name
    ·The full name of the nominee
    ·Your daytime contact telephone number
    ·Your email address
    ·Your address, including postcode
    ·Why would your nominee be the perfect person to participate in the game on the programme (include as much detail as possible – maximum of 200 words)

    Nominations must be received by the closing date, 17 March 2008 at 11:59 p.m. Nominations received after the closing date and time will not be valid.

    Nominees will be selected to participate in the game on the programme according to the online nomination which the Promoter considers to be the best in their sole discretion, from all the eligible and valid nomination forms received by the closing date and time. The person who nominated a nominee will be contacted on the phone number and/or email address they provided at the time of submitting the nomination form. Reasonable efforts will be made to contact the nominating person. If such persons cannot be contacted, or if the nominee and/or the nominating person is not eligible and/or does not comply with these terms and conditions as specified, then the Promoter reserves the right to select the next eligible nominee and thereafter until a participant is found. Not all nominating persons will be contacted and not all nominees will participate in the game on the programme.

    By nominating someone to participate in the game on the programme, such person accepts that if their nomination is successful, the producers of the programme may turn up at their house or at the nominee’s house, or at a venue chosen by the Promoter, live on-air during an episode of the programme at any time during the programme series. If a selected nominee for any reason does not agree to participate in the game on-air in the programme, he or she will not be able to participate further in the game and will forfeit all rights to the prize. Nominees who participate in the game on the programme must comply with any directions given to them by the programme, without limitation, in regard to conformity with the Ofcom Broadcasting Code.

    Cost of nomination:
    There is no charge to make a nomination online, but your internet service provider’s fees may apply to entries via the internet.

    General Terms:
    ITV, the producers of the programme and/or the game and their sub-contractors do not accept any responsibility whatsoever for any technical failure or malfunction or any other problem with any telephone network or line, system, server, provider, software or otherwise which may result in any nominations not being properly received or recorded.

    ITV cannot promise that the website will be free from errors or omissions nor that it will be available uninterrupted and in a fully operating condition. The website may be suspended temporarily and without notice in the case of system failure, maintenance or repair or for reasons beyond the reasonable control of ITV. ITV will not be liable to you or to any other person in the event that all or any part of the website being discontinued, modified or changed in any way.

    Nothing in these terms and conditions restricts your statutory rights as a consumer. For more details on your statutory rights you should contact your local Trading Standards Office or Citizen's Advice Bureau.

    1. Your Name
    2. Your email address
    3. The IP number of your computer
    4. The date(s) on which you submitted your nomination and experienced problems

    7. A contact telephone number you can be reached on, if necessary
