Personal details (the form is missing some required fields)

I have checked everywhere and no form is missing fields.

I registered ages ago, and then reregistered for the matter box aimed at my age group, didn't get nowt. Won't be bothering again.

I registered over a year ago, tried to log in today and it didn't recognise me so i had to reregister! don't think i'll be getting the latest box then!
