Thanks for the 2nd link, tried that too and it says page does not exist. Is it me do you think? Can you open this and see the form?

It works when I click on the link, but maybe thats because I posted it. It needs someone else to try it, then we will find out for sure if its the link thats not working.

Yes I tried that too but it just brings up a blank screen, maybe it was only on for a few days. If you could attach a copy that would be wonderful and much appreciated, or do I need to send an email address?
