Cover for indemnity form ??

I have too apply for a new indemnity form to retrieve some shares for which i dont have the certificate, Captia registrars are charging for this service which i dont have a problem with, but this extra i am a little curious of as they also require "Cover is needed to support the indemnity form"..which they charge another 40 quid for, I am not really sure what this is or if i can sign it myself, can i source this elsewhere ? Thanks ..Andy
