insureandgo.co.uk are pretty reasonable - worldwide inc usa and canada, ecomony single trip insurance, no pre existing conditions £22.05 each. They also do silver £32.75 each, gold £40.92 each and annual policy £40 each.
I've used them quite a few times and my daughter used them for backpacker insurance and she was able to have £0 excess - when she was ill in Kenya they were marvellous.

Worth checking out https://www.insurefor.com ; https://www.cheaptravelinsurance.com or https://www.flexicover.com are other possibilities.so too those issued by American Express.
But above all choose an insurance policy that has full cover for USA (minimum £2 million preferably £5 million medical expenses cover plus full "GetYou Home" cover AND one with a 24 hours emergency service and a North American office/contact number since many US hospitals (especially outside the main hospital groups) will NOT invoice "out of country" insurers . Cheap cover should only be an "important factor" in your choice .
