Here is an example where someone was drafting it themselves for the solicitor to sign. Though this example is for a trustee company as opposed to an individual.

The other aspect to consider is that any name or address changes from what is written in the will might need to be referenced.

EDIT: clicking on the date seems to allow access to the thread.

The declinature document needs to be docquetted in a similar way to the will docquet.

You could post your redacted para 2 declaration if you want comment.

    I have put this

xxxxxxxxx Executor Nominate of the deceased conform to the will of the deceased dated xxxxx  which is produced herewith, docquetted and signed by me as relative hereto and that i am the daughter of said deceased.

xxxxxxxxx and xxxxxxxxxx have declined to act as Executors,Conform to the Letters of Declinature dated and signed which is produced herewith

.In said will xxxxxxxxxxx wife of the said deceased who was appointed executor nominate primo loco in said will, predeceased the said deceased.

    For the declinature

"[Place of signing], [Date of signing]This is the Letter of Declinature referred to in my declaration of this date to the Inventory of the estate of the late [Full Name of Deceased].[Signature of the Acting Executor/Applicant]"

Your name does not go into para 2, it goes into the box at the top of page 2 (along with your address).

Below is a suggestion for a restructure of para. 2. I have assumed that you have two separate letters of declinature with same date. Also assuming that there are no name or address differences in the will that need to be referenced.

Names and designations need to be as stated in the will.

    That I am

daughter and executor nominate of the deceased conform to the will dated xxxxx. Xxxxxx xxx the wife of the deceased who was nominated executor in said will, predeceased the deceased. Xxxxxx  xxx and xxxxxxx  xxx who are also nominated executor in said will have both declined to act conform to letters of declinature both dated xxxxx. Said will and said letters of declinature are produced herewith, docquetted and signed by me as relative hereto.

The addresses were different from will but that was added to minute of declinature for both solicitors . What do you mean by names and designations ?? Think the date on letters are a day apart but will add this
