Moneysupermarket, for example, clearly specifies anything in the past 2 years where you've seen a doctor, and anything serious from the past 5 years. Seems pretty clear.

Not dealt with any that did not clarify what you need to declare.
If you are not sure then phone the company and ask.

The "ever" tends to be much more significant things like cancer, heart attacks, strokes. More minor things tend to be restricted to 1-5 years with a bigger concentration around the 2-3 year mark.

Moneysupermarket is just a comparison site so its results may not be totally accurate for every insurer. Just view their results as a start point.

The OP is talking about comparison sites (see first post) which is why I mentioned them.

All sorted. After some time spent digging, looked at a site which was more definite in their wording of questions (moneysupermarket, which is my usual starting point for car insurance but hadn't looked at at time for travel). At least had some light relief (OK, I'm easily amused)in search finding the shortcomings of one insurance company's AI when it couldn't give me an online quote as it simply kept repeating that I'd declared one condition twice. Turned out their computer thought that a knee and an ankle are the same thing.
Still think the way some questions are worded sometimes, doesn't always lead to an entirely accurate medical picture but so long as it suits the insurance company😊.
PS - ended up sticking with my existing insurers as best fit & price

It doesnt but this is mass market consumer insurance and therefore it's a numbers game.
The more questions and details you ask for the less people bother completing the forms and so leave and go elsewhere. We once had a long debate about asking a cars current mileage (for motor obviously, not travel) and just that one item was seen as a major roadblock so wasnt supported.
I'm sure they could more accurately price if they asked when you were first diagnosed, when your last appointment was etc but many won't know and if strictly applied you start declining claims if they guesstimate rather than spending time to find out the correct dates or values.
Generally it's considered better to sell 200 policies with roughly correct premiums than to sell 3 policies with more accurate premiums.
