hat I read the OP only added two people so with himself Woukd be three.
There is no suggestion of there being more than three people-( OP + 2 friends ).

I thought drivers had to be using it for your benefit, not just because they let you live there rent free.
Did you get a letter confirming they were added? Did it list the drivers. Do they have your current address?

How do you know there was only one named driver to begin with?
The limit of three and the removal occurring at the same time as two additions is a logical explanation for what could have happened. Hence, this is why the op has been asked.

No, but the OP has also failed to respond to the comment. It's one of the most obvious causes of the problem because of the limit. We know they have a recent ex so there's an obvious person that may be named on the car and the OP hasn't taken them off yet.

Yours is based on you assuming something that so far has not been confirmed.

If you're on the Motability scheme, please check your Certificate of Motor Insurance to make sure YOU are listed as a named driver on your own policy.
If you have any policy, not just Motability, check the details shown are correct especially after you have made changes, just common sense. Mistakes happen from both sides and it is your responsibility to check everything is in order.
I can't see this going anywhere with the ombudsman because the paperwork issued, both hard copies and digital, would have clearly stated who is entitled to drive and when carrying out any changes there is always a covering statement to check that everything is correct.

So both suppositions are based on what has been said. You are assuming there was only 1, and that has not been confirmed so far.

I guess we are not going to find out and the best guess remains that the 3 driver limit was the issue
