That is not the full text of the BVLRA standard.
Dents up to 15mm are acceptable, up to two per panel.
BUT, dents to the roof are not acceptable.
They will always be charged for.

They are writing to you because it's a regulated credit agreement and so they are obliged to tell you about your right to go to the FOS. It doesn't actually sound like a final response.
Regarding a) and d), they won't repair the van. End of lease vehicles get sent off to auction. A van trader looking at it will think it's a nice van, except for that dent in the roof which will cost them about £400 to fix. They will therefore bid less to allow for that.
If it sticks, force it.
If it breaks, well it wasn't working right anyway.0
