DVLA SAR Template - Trying to locate one that requests all relevant data

Been searching the forum for a template that lists out all the relevant information that should be requested in the SAR Form, but not been able to pin down anything I thought was succinct to use.

( I understand that it may be tucked away somewhere and I've not found it or "all data" is ok to use but I'm just not sure)

I ask as I was going to simply ask for all data, but then read about V888 forms where a PPC will not use the automated KADOE API's to obtain the data.

I figured putting in specific items

The search term I used was…

"DVLA SAR Template"

and figured the best thread would come up, but I'm 3 pages in and not found anything that answers this specific queston.

Are there any guides or pertinent information that provides a clear piece of text to use when making a SAR from the DVLA?

    This is one I’ve been suggesting for a good number of years now. Seems to work for the information requested. If you want other bits and pieces, just tag them onto it.

    You should email the DVLA and ask which organisations (when and for what reason) accessed the registered keeper's data from them between a range of dates which includes the date(s) of the parking incident(s).

    You need to provide the registered keeper's full name and address, the address on the V5C logbook and the Vehicle Registration Mark of the vehicle involved in the parking incident. 

    SubjectAccess.Requests@DVLA .gov.uk

    This service is free of charge. 

    Even though you email your request, the DVLA will respond via Royal Mail.
