How often do retailers actually fail to process refunds?

Curious about other people's experiences with this.

I returned 3 items to ASOS in November - just did a proper audit of my statements and only 2 refunds came through. £47 short. I've got the return confirmation emails but no corresponding refund for one of them.

Going to chase it but got me thinking - how common is this actually? I've been assuming refunds just arrive automatically but now I'm wondering how much I've lost over the years by not checking properly.

Anyone else found missing refunds when they've actually looked? Or had to chase retailers?

I track my spending pretty carefully but returns are a blind spot - different cards, different timescales (3 days to 3 weeks), partial refunds vs full refunds. By the time I think to check, I've forgotten the details.

Interested to know:
1. Have you ever caught a missing refund?
2. How much was it for?
3. Do you have a system for tracking them or just trust it'll arrive?

Starting to think the 6% "lost refund" stat I saw somewhere might actually be real.

Since I do maybe one return every couple of years I'm pretty sure that my unnoticed return failure rate is zero.

How many returns are you doing (and why) if you can lose track and not be sure if they've all processed OK?

On the other hand they sometimes forget to charge in the first place.

I never got a refund from Motorola for £700, Lloyds processed a Chargeback and in a few days time the 30 day period for challenging it will be up, not that I expect it to be challenged. I keep track of all my transactions so I can't imagine why I would not be aware of a missing refund.
