PCN by Civil Enforcement Limited (CE-Service) - Failed to obtain permit

Received a great present in the post by the lovely Civil Enforcement Limited.
Attached photos of the PCN
Took my friend to the hospital dropping them off at the entrance and waited, it's not a car park with barriers just a one way system with parking along the route. Was there for 35-40 minutes total roughly. I think it was a case of me needing to put my registration inside, the place is over an hour away from me so I can't go and check the signage.
I just wanted to confirm which template to send before appealing as seen a few templates from other posts as well as the NEWBIES thread one, would either of these work or just stick work with the original template.

I dispute your 'parking charge', as the keeper of the vehicle. I deny any liability or contractual agreement and I will be making a complaint about your predatory conduct to your client landowner.
As your Notice to Keeper (NtK) does not fully comply with ALL the requirements of PoFA 2012, namely paragraph 9(4)(b) in that it was not given within the "relevant period", you are unable to hold the keeper of the vehicle liable for the charge. Partial or even substantial compliance is not sufficient. There will be no admission as to who was driving and no inference or assumptions can be drawn.
I suggest you go pursue the driver or cancel the PCN.

You must include a close up actual photograph of the sign you contend was at the location on the material date as well as your images of the vehicle.

Either, or none. I'd ignore them because there is no appeals service that considers these cases fairly. Come back if you get a claim.

Thanks, went for the appeal route and waiting for the appeal email to arrive... slightly different to the form option on the website.
Trying to find the hospitals best email but it's a private one by The Circle Health Group1 -
Told to call the main reception on Monday, will update soon as I know2

CEL wont cancel it, meaning that no appeal will work ! ( plans B & C. )
Lets hope that your complaint on Monday bears fruit, so plan A, a cancellation by the private hospital2 -
