The question asks "are you currently suffering from, or have ever suffered from any of the following? Please tick where appropriate and provide further information below"....
    High Blood Pressure is listed
Tick the box and say it’s well-controlled in the free text box (assuming of course that statement is true).

There’s really nothing whatsoever to worry about.

Fair enough, what further information is needed if you tick that box?
It just says if you tick any box to provide further information - so I can just put that I am taking medication for it, or I can put its being managed by medication - then there is a notice at the bottom of the form if I provide false information I can be dismissed instantly.

You're really over-thinking this - I was diagnosed with high blood pressure several years ago and it's being managed really well by medication.  It wouldn't bother me at all to tick the box and say exactly that in the section provided.

There are millions of people in the UK taking blood pressure medication, don't feel embarrassed or the need to lie about it.
