I did once hear a friend responding to a call where she was misidentified by title, firmly telling the person on the other end that she was MISS LastName, not Mrs. I suspect there was already a flag on her name to prevent anyone addressing her as FirstName. 

It's ok for the avoidance of doubt the employer wants everyone asked how do you wish to be addressed.

I have emboldened the  strange syntax  that is likely to cause  you  issues   with  many people  

i'm blunt and direct in  interactions with colleagues, especially about safety critical things    

however your last statement  there   will be read by the  vastest  majority of people in one of three ways , all of which are problematic 

1. arrogant 
2. flippant 
3. condescending
