DLA refusal and MR

My daughter has got refused DLA. We received the decision back on 27th August. I have been in contact with a benefit advisor within a charity for her condition (she has Glycogen Storage Disease). I requested back statement of reasons and then the DLA completed form back (this took 3 phone calls, as none of the forms were returned to me).
I am wondering what my next steps should be. I know it’s 4 weeks for an MR request but it can be requested within 13 months. What advice do you give? Request an MR via phone, giving my reasons for a late request or complete the MR letter, giving the reasons on paper. Or complete a new claim.

I always request the MR in writing outlining exactly where you feel they were wrong and how you feel she meets the requirements. You can't really do that over the phone1
