C1 Confirmation - Help Please

Recently submitted confirmation application and it has come back with 1 post-it note on the first page saying “name of deceased differs to that of the will to c1”.

the will has both the deceased maiden name and married name. The way it is written is first name and then “maiden name” or “married name”. I have filled out the form with the deceased married name. 

Does this mean in box 3 surname I write “maiden name or married name”?

would the rest of the form have been reviewed or would this have stopped it? It is all that has been highlighted on the form and letter but I’m worried they’ve just stopped at the name and I have fell at the very first hurdle!

                  In said will the deceased is named [name as on will].
