C1(2002) Grant of Confirmation - sole (executor & beneficiary) have (will & survivor clause)

I filled as below, however, unsure about them...
Page 1
Your reference - Wife of [deceased full name]
Page 2
Declaration by - apart from name and address, I added: In the said will I am named as [exactly the same name as the will] - my IDs and the Will's surnames order are different. Also I added: bear [commonly used name/my most known name] - omit my maiden name.
BOX 2 That I am...
the Wife and Executor Nominate of the Deceased conform to the terms of his Will dated DAY MONTH 2025 which said Will is herewith produced signed and docquetted by me as relative hereto and wherein the Deceased is designed as Mr [deceased full name] and in said Will the said Deceased is designed as residing at [address postcode] additionally in the said Will I am designed as [exactly the same name as the will] and as residing at [address postcode] but I also bear the name [commonly used name/my most known name].
Page 3 (Inventory)
HERITABLE ESTATE IN SCOTLAND: NIL (all filled in the second column) - no address no MID number because survivor clause ? Does it sound ok ?
Notes to help you guidance (Appendix 2) Including pension benefits for Confirmation on form C1(20020 says about lump sum and refund of contributions. These conditions seem for me to fill in the form as they say they are part of the deceased's estate however, I've never seem/heard anyone filled in about Lump sum and refund of contribution, so far ? I understand refund of contribution, refund insurance etc..., are deceased estate however the Lump sum as well ?
Page 4
BOX 14 Guidance mentions liabilities = house hold bills - Utilities. Even we were living together, do I need to fill in the figure which maybe the most recent bills before the death ? We were paying our utility bills from my husband's account.
Page 5
BOX 21 Yes (because survivor clause?)
BOX 22 No (because I'm a surviving spouse?)
BOX 23 Total Estate for Confirmation add half the value of the house
BOX 25 zero (all the estate passes to me & no children)
Page 4 BOX15 & Page 5 BOX 16: already '0' and 'tick' are filled in and I can't change/edit them. I assume I can just manually cross them out with double lines. Is it ok ?
Thank you very much for your time and all your help.

deer3 said:I'm wondering if anyone help me to fill in the C1 form, please ? I am a sole executor, sole beneficiary. I have will and survivor clause. My husband had no siblings. We have no children. Bank asked me to obtain Grant of Confirmation to release my late husband's fund. I filled the documents referred to Notes to help you fill in form C1, this forum also a couple completed C1 forms (however both are different scenarios for me) made by solicitors in the past.

Thank you very much for your time and all your help.I am not totally clear about the name and address issues.
Was your husband’s address at the date of death different from that stated in the will?
Is the name of your husband as stated in the will his correct name?
Is your address different from what is stated for you in the will?
Is your name as stated in the will correct?
Are you known by another name?

Hello buddy9, thank you very much for your reply and your support. I'm sorry I couldn't write clearly. Please bear with my English. Only my name is the issue. Answers are...
1. No
2. Yes
3. No
4. Yes, however, different form my passport and driving license (I'm not British therefore I state double surnames? (sorry I don't know how do you say) for these IDs for my convenience. it's easier to use them between two countries)
5. No
Order of my names are...
Will : GIVEN name my husband's surname (my maiden name)
IDs : GIVEN name my maiden name my husband sure name / GIVEN name my maiden name (my husband's sure name)
I can provide our Marriage Certificate.
Thank you very much.0 -
Here are some suggestions:
- Page 1 Your reference - Leave blank
- Page 2 Declaration by - Enter your name and address the same as used on page 1 of the C1 and add “otherwise known as [insert alternative name(s)]”
- With no name or address differences in the will then the entry in the box in para 2 would be simply:
That I am
the wife and Executor Nominate of the deceased conform to his will dated [Day Month 2025] which is herewith produced, signed and docquetted by me as relative hereto.
- Page 3 (Inventory) - If the house has a survivorship clause in the title your approach is correct. The house share is not listed in the inventory but the share value is included in the calculations on page 5.
- If the pension lump sum is a discretionary payment and is payable to you without the need to exhibit Confirmation, it should not be included in the inventory.
- Page 4 Box 14 no need to include anything about utilities
- Page 5 Boxes 21, 22, 23, 25 proposed entries ok. Also needs an entry for box 24.
- Manually strikethrough the offending entries that cannot be edited.
