Private hospital sent invoice to insurer 2.5 years later

Hi all. Tried searching the forum but haven’t found anything that similar but I had an ultrasound which I was authorised for with my private healthcare.
The hospital has now sent an invoice to my insurer 2.5 years later!
My insurer has sent it to me saying

we require original invoices (which must be forwarded to us within 90 days of
treatment) other than where the hospital or consultant bill us direct by electronic
means, this is except for claims for Cash Benefit. Any invoices received outside the
aforementioned 90-day limit will be returned to you to settle direct with the provider
of service.
Regrettably, as the invoice was received outside of the 90 day limit, please contact Hospital, to arrange direct settlement of the outstanding amount.
The hospital must be at fault here.

Im guessing I have to pay it- has anyone got any advice or had this issue or something similar?

How much is it for?
Have you spoken to the hospital to find out why its taken so long for them to bill?
Was this all done with your insurer pre-approving everything, it being one of their standard suppliers and doctors etc?0 -
It’s for around £400 glad it wasn’t thousands. Everything was pre approved by the insurer. And it was an agreed doctor / hospital etc
i have yet to call the hospital or insurer. It’s only just been sent to me by the insurer, today.0 -
Call up insurance and now they have said another invoice is coming for £2,500 for a separate procedure 2 years ago!0

Do you have legal expenses cover on your home insurance? If so, I'd be inclined to use it to see if any assistance can be provided. The hospital obviously needs to get their administration systems in order as it seems totally unfair to leave it this length of time to produce and send out an invoice.0
