    If you did get a social housing property, she may be able to succeed the tenancy but not the property, so she would then be expected to downsize. 
    I'd agree with this: the supported housing route will be a long slog, but it's probably the best you can do for future-proofing.

    Your local authority will have information on their website about care act assessments, and she can self refer. You don’t need to go through citizens advice
