What kind of instructions are usually added to both kinds of LPOA to ensure your wishes are met after you lose that ability to manage your affairs

    the main one for the health and welfare one is your decision as to whether you allow attorneys to refuse life-sustaining treatment. (FYI if it has got to that point and you are over a certain age, I'd suggest allowing it as there is no magic to pain relief) No-one is going to knock you on the head despite what the anti-assisted dying liars say, you'll just take longer to die.

    you can give more detail about circumstances - an 'advance directive' is also a good idea for that.

    financial attorneys are obligated to spend your money in your best interest. Where it goes after your death is what your will is for.
