The timing of the policy and declarations needed become moot if the insurance doesn't cover cancellation due to being pregnant at all (vs say cancellation due to a pregnancy complication).
You need to check your insurance policy for that.

There was a recent thread about this whereby the insurance wouldn't pay out for cancellation due to late stage pregancy but also wouldn't cover complications due to flying against medical advice - a catch 22. Might have been this one, haven't read back in full https://forums.moneysavingexpert.com/discussion/comment/81516050#Comment_81516050?utm_source=community-search&utm_medium=organic-search&utm_term=insurance+pregnant

I'd note however that OP's case of 20 weeks is very different to late stage pregnancy.

Would insurance providers cover any conditions if the policy holder travels contrary to medical advice ?

Possibly if the claim is unconnected to the reason medical advice warned against travel.

Thanks for that. Indeed there is no "one size fits all" scenario.
FCDO advice in respect of certain countries is not to travel at all, unless essential - or not to specified areas within these countries - for security reasons rather than potential health hazards.
I wouldn't be risking an unborn child by going to such locations, however that's my personal opinion.
