SDP and IS10 form

Just providing a bit of information when requesting the SDP to be added(if eligible).
You need to complete an IS10 form.
I have literally just got off the phone with DWP and they said I have to complete an IS10 before they can look at the request.
He asked if I could call them tomorrow, as he didn't have access to the form, so they could complete the form tomorrow!!
Long story short....he got his manager to email the form and he completed it with me.
I am flabbergasted beyond words!
