Council Tax SMI and moving area

My father does not pay any council tax due to a CPN completing a form for him and getting the SMI exemption. This has been the case for just over two years.
He now wants to move into a smaller home but in a completely different area that will include a different council, new GP etc which will take time to register.
Will the new council need a new application completing by the new GP or is it possible to take written confirmation from the old council and continue the exemption this way?

I imagine it will be a new form/ application as all councils operate their schemes independently. Do you have copies of previous application? No harm in asking new council if they will accept old docs you have.1
