It should be declared, as it is a PEMC, regardless of whether it needs treatment in the future.
I have complex PEMCs and almost all insurers decline to cover, even specialist medical insurers. The only one I have found that will cover me is AllClear. They actually allow you to exclude an undiagnosed condition from cover, so may allow you to exclude a diagnosed conditon that you do not want to be covered.
That said, it is risky given he is doing a non-standard trip. An accident is exactly that, an unexpected event that you didn't anticipate happening. If your son has an incident that affected his knee, it wouldn't be covered. The only other option is to self-insure if you can afford it or are 100% confident it till not be an issue.
