If you don't currently have a Capital One Account & you haven't applied for a Capital One Account within the last 6 months then you might like this little code I have here:

This offer's 18 months 0% on balance transfers, followed by 7% for the remaining time on balance transfers.
It appears to only be valid from the postal form I have received, but you might want to try telephoning 0800 952 5452 explaining that your form has been damaged / destroyed & they may process your application over the telephone or post you out the relevant form.
