Millets/jd sports missing item from parcel and legal form

first post here. Issue is Millets owned by JD sports is regarding an online order I placed which has one item missing from it. It’s taken me ages to get hold of someone to deal with this, two items were due to arrive a cost and a rucksack, only the cost arrives. On finally getting hold of someone they say they have to investigate first and are refusing to issue a replacement item until I sign a legally binding form with the following wording, which at present I refuse to sign as I would be lying if I did so!

I confirm this declaration is true and accurate in all respects.

 I agree to co-operate in any investigation into the missing items with either the carrier service, jD Sports

Fashion PLC or the police

 I understand making a false statement could potentially lead to prosecution and all forms of

communication will be provided to the police.

 I understand making a false statement would lead to JD Group attempting to recover any losses using a

3rd party civil recovery business.

 I understand making a false claim would lead to JD Group refusing any future online orders.

LEGALLY BINDING DECLARATION OF NON RECEIPT

 I wish to deny receipt of the parcel tracking number: ..………………. ………………………………………

 I hereby declare that the above-mentioned parcel has not come into my possession and has not come

into the possession of a person known to me.

 I can confirm that I have checked with those within my household & neighbours who may have signed or

received the goods & the parcel has not been received.

 I have fully checked around my property and have not located the parcel.

 I will inform you immediately if the parcel arrives.

    You might have to use a little common sense here.

    If there are any statements you cannot truthfully agree with, amend them.
    For example:
    • I wish to deny receipt of the parcel tracking number: ..… Eurohike Nepal 85L rucksack 
     I hereby declare that the above-mentioned parcel  rucksack  has not come into my possession.

    In principle, promising to tell the truth is not unreasonable. I think JDSports will simply not proceed further with this until you agree to co-operate with their investigation.

    Simply amend the declaration form as @Alderbank suggests.   (ie amend it with the correct details as applicable to the item you did not receive - do not use the example rucksack given by alderbank unless that is the item that you did not receive).

    Make sure you keep a copy of the signed declaration
