Budget friendly path fix

Hello , I am hoping for some ideas please or even info on what not to do with my bungalows front path. I am wondering if there is an affordable 'solution' that I dont know about. Building repairs and DIY are not something I have knowledge off.
I have a concrete ramp that was made almost 15 years ago as a disability access path. The builder made a frame, added rubble then poured concrete in coming back to add on handrails and remove the frame.
As a wheelchair user who is also ambulatory for short distances its important the path is smooth unfortunately its weathered rather badly. There are patches with dips (pot holes) and rough bits of concrete coming away. We did patch it up with a bucket of some concrete a few years ago but that's lifted now. With my poor walking and ability to drag toes, trip over small ledges this is not ideal.
I don't have much of a budget so taking it away and starting again isn't an option (i dont think). I cant paving slab it as then the door threshold and house floors would be too low .
I don't know if its normal for patch to wear like this or just another problem from the company who did the work.

Photos, please
There are lots of products out there that could do this, but it depends on how bad the damage is.Also, there are decorative coatings that can be applied afterwards to make it all look the same.1 -
Yes a photo would be really helpful.
The slope (we can enlarge it on our gadgets to get a close look) and where the slope meets the door threshold so we can see what you have to play with.
I have a slope installed for the same purpose but was there when I bought the bungalow. They came up with an ingenious idea if a bit Heath Robinson 🙂I can rise and shine - just not at the same time!

There isn't much of a slope on the ramp, not really sure why it counts as a ramp not a bridge.
