Probable very few since the advent of online banking, and the ending of cheque books.Looking at the RBS website as an example you set up the joint current account and then change the instruction so it needs both of you. This removes the ability to use telephone mobile or Internet banking and you can’t have a debit card.
Other banks may do similar.

As above banks could offer it but because they are expensive to run and the potential liability if one signatory manages to withdraw funds they will not.0

Well if it is the case that banks don't offer the facility it is madness I probably will need a cheque based one as I can appreciate that online banking would not work

Thank you for your input it seems that Lloyds do still offer this option. Which is great. That has saved me calling endless banks1

Is that an assumption about the 'old cousins'
The cousins (through a marriage, so more friends really) are brothers required by court order to operate the account in this way.
It was set up a few years back following some contention over a will regarding a late executor of an executor.
It is just a standard account to which certain payments are made and from which payments are made by cheques signed by both.
