A failed injector would have resulted in rough running from running on 3 cylinders. Also it ought to have illuminated the EML (but I'll give you credit it might not have in this instance). So you had the warning. If it were stopped soon after the rough running, the engine wouldn't have been further damaged.
And yes, you're well out of the 6 month period in CRA2015, so I think this one is your responsibility to fix.0 -
