Adding illnesses to the ESA to UC Migration form

Hello,  I have filled in the online form to migrate from ESA Support Group to Universal Credit, but have realised I forgot to list Diabetes among my illnesses in the Health page.  

Is there a way I can add this, and do I need the specific date of diagnoses?

Also, they list the methods of id they accept, I have a drivers licence, p60 etc, but on the webform they ask for 2 different id's from a different list.  I only have the drivers licence from that list, but I used the government id app for road tax, why can that not be used?

    As you are in the ESA SG you will be migrated to UC LCWRA so not listing your Diabetes doesn't matter.
    You only need your drivers licence (as long as it's a photo) from the first list, so you need a proof of address, a bank statement  council tax bill in your name.

    The above four items are all that are accepted to confirm your ID online; in person, you can use other items from the Gov list.

    I believe there's a section about adding further information about conditions and disabilities. I don't believe anyone with a medical background looks at this information anyway, so if you think it will help, submit it. Remember, they're aware because they're asking you to migrate. You can also change this once it's submitted by reporting a change of condition. Or just add journal notes if you think it helps.
