PCN due to paybyphone app outage - Britannia parking now DBCL- Directions questionnaire

Summary so far - PCN issued July 24 for parking and waiting in car park (21 mins between entry / exit) trying to pay using paybyphone app which had a full platform outage at the time, the only other type of way to pay was a coin operated machine and I didn't have any cash. Left the car park to seek other parking as was now late for an appointment. Was issued PCN. Appealed as paybyphone app confirmed full system outage. I even paid for an hour of parking on the paybyphone app that I didn't use a few weeks later to cover any losses. My appeal was rejected by Britannia parking who failed to notify me. Next I hear is from DBCL a demand for £170. Have not paid. Have filed my defence using template / similar from newbies thread. Have just recieved directions questionnaire. Not sure what I should be writing on it? Is there an exemplar filled in on the threads somewhere? Specifically help with:
Suitability for determination without a hearing?
Mediation appointment? It says if I don't attend a mediation appoitnment I may be subject to penalties.
Hearing? Experts and witnesses? I am not sure what to write.
Thanks in advance for any replies and help. This has been the source of a lot of stress and has dragged on nearly 12 months now. They are quoting that I will now have to pay over £300 if this goes to court.
