I bought a quite expensive coat from eBay (over£100). It was damaged. Paid by PayPal. The seller said she would accept a return. I have done this and it said I would get a refund when the item is received back. I understand that the procedure is for the seller to refund then issue you with a postage label? Is that correct?
my concern is that if I send it back without a refund she will not refund.

Open a case for SNAD/Damage on ebay, make sure you open that sort of case and not a change of mind. Then ebay will ask seller to send you a pre paid return label, and if they don't, then ebay send one and charge the seller. Pack item and return and once it shows as delivered, or refused by seller, you will get your refund within 48 hours.
