Completing C1 for Paisley Sheriff Court


Hello all,
I’m completing the C1 form regarding my mother who passed away from cancer quite suddenly last year. She had been living in the family home and, following some time in hospital, received a week of end-of-life care at a local care home where she passed away.
I’ve had the form returned from Paisley Sheriff Court twice now, but have found this forum a bit of a godsend to get this far.
The corrections I have to make are:
- The Deceased’s address is noted as residing at [Care Home] in box 8 on page 1 of the confirmation [which is place of death - my comment]. If this is her latterly address is should be used in the Confirmation and her former address referred to in the declaration.
- The Original Will should be docquetted, dated and signed.
In the first submission, they rejected the following wording in page 2 section 1.
Looking at things again, I think I should use that text again in page 2 box 1, but have the Care Home address in Page 1, box 4.
The issue is that my mum was not formally a resident at the Care Home. It was a little over a week of end-of-life care, following several months in hospital, and there was no time to arrange formal admittance et al and any connected paperwork regarding council tax etc etc.
I still think I should use the rejected text again in page 2 box 1 but have the Care Home address in Page 1, box 4. Would value anyone else’s perspective though!
I am a little uncertain as to exactly where I’m supposed to write the docquetted statement on the will and what it should be. For what I’ve been able to gleam from this forum, I am thinking:
At Glasgow [DATE]. This is the original will referred to in my declaration of this date relative to the inventory of the estate of the late [FULL NAME].
[My signature]
Would value comments/advice, particularly for anyway who’s been successful with Paisley Sheriff Court.

One weeks EOL care in a care home does not constitute a permanent address.
I would enter the family home as the address in page 1 box 4. And make no other references to her address. (could that mean a different court?)
You have an incorrect spelling of sheriffdom.
The will docquet looks ok - though the word ‘original’ is not needed. The docquet can be written at the top of the first page of the will if there is space.

I'm wondering if I should just use the family address in both p1 box 4 and p2 box 1 and spell it out in the accompanying letter? There doesn't seem to be any way I can provide context in the c1.0 -
is the family address the same as the address stated in rhe will?0

Only enter your mother's full name on the box in page 2, do not include an address here.
I would suggest that if you want to reference in a covering letter only say that the deceased was not resident in the care home and that you have now amended the C1 to correct the address issue. Do not elaborate further.1 -
@buddy9 I've had a letter through this morning telling me everything is ok. Thanks for all the help getting it over the line!
Of course the letter is needlessly snarky but that seems to be par for the course with PSC.
I'm being asked if any additional certificates are necessary. It was a fairly basic estate and the accounts are with one bank. I'm not envisaging a situation where I would need more than the one I will be issued. Perhaps one additional for backup?

A ‘certificate of confirmation’ is supplementary to the Confirmation document. A certificate certifies that confirmation has been issued and includes reference to only one specific item on the inventory.0

Thanks! My assumption then is that I won't need any additional certificates and can use the document I get from them to progress things with the bank. See below for the wording of today's (rather passive aggressive) letter.
