DWP Chasing Overpayment Incorrectly

I need advice on a situation with the DWP regarding my 90-year-old grandfather. Recently, he received letters stating he owes £16,621.99 due to overpayment from 12/11/12 to 02/10/22. This stems from a period when he was hospitalized in 2012, and my grandmother received the Underlying Carers Premium, which enabled him to get pension credit. However, he was never formally informed that his pension credit would be reduced or that he needed to reinstate it after returning home. My grandmother continued to care for him until 2022, when I took over and started receiving attendance allowance, which triggered a review. We believe the DWP failed to communicate any changes, and now they are chasing a debt from over 10 years ago without providing evidence of correspondence. At 90 years old, with my grandmother being 86, this situation is causing significant stress for them. I have sent a Mandatory Reconsideration letter but any guidance on how to handle this would be greatly appreciated.

    This seems rather muddled. 

    Your Grandfather and Grandmother jointly claimed Pension Credit but your Grandfather was hospitalised in 2012…? How long was he in hospital for?

    Is the overpayment JUST Pension Credit or for Attendance Allowance too?

    You are now the one receiving Carers Allowance I assume? (Attendance Allowance is paid to the individual, not their Carer). 

    I’m assuming, but this is reading between the lines - that Grandfather’s AA stopped (but was never reinstated?) due to hospitalisation but neither party told Pension Credit this?
