Thanks for sharing.
I was very confused I only had two questions to start with and nothing else.
I scrolled down but no more questions or a next/submit button.
I clicked tab on my keyboard and it brought up this second scroll bar within the page and showed me this question and more.
I just thought that was worth noting if anyone was having the same issue.
