Fraud - Blocked Card Issue

Has anyone ever had a situation where bank fraud have 'blocked and cancelled' a card, which was subsequently replaced with a totally different numbered card, but the original was used in error, by me, to make a purchase which progressed as normal?
Last week I inadvertently used a 6-months-ago-fraud-blocked TSB card number when ordering off my phone as it was a default card number. To my dismay, the transaction went through! Now TSB say I do not need to worry!
How worried should I be? Is this a possible cover-up?
P.S. A formal complaint has been raised with TSB by me, as neither telephone banking chat or internet banking chat gave me any real confidence in what they were saying about how it could/did happen.

Any card replaced as lost/stolen (which includes fraud)
Always have different card numbers. Or it would be pointless stopping card.

Surely you were trying to complete the transaction, and you managed to do it.0 -
If it happened as you say then it's a clear security breach. You're right to complain, but of course you won't get any compensation because you've suffered no loss.0

According to the bank the replacement card was the only valid card against the account - yet the transaction still progressed under the old, cancelled-for-fraud card number.
Please enlighten me as to what a CPA is in the UK. According to Google it is a Certified Public Accountant which I cannot see has any context here.0 -

Of course I wanted the transaction to go through - I just did not expect it would go through on a cancelled-for-fraud card which was cancelled by the bank - not me, the bank - 6 months ago.0 -

Not complaining to get compensation but to make the bank aware there is clearly a security breach.
This transaction was for £60, but what if it had been for £600, or £6,000, and had not instigated by me? IT should never happen on a cancelled card.2 -

As well as not being stopped by security the merchants will be given your new card number to use in the future automatically. The intent being that you dont get stopped for driving without insurance etc because your bank has had to issue you with a new card number due to fraud.1 -
