Driving licence after ban

Please help as stressing out.  I need to sort licence for fantastic job opportunity.  

Basically back 9 years ago was caught drink driving (minimally over limit) had 9 month ban.  Never renewed licence since due to various reasons, mainly lost job, no car, not needed haven’t been driving.  However now wish to sort licence out, old photocard expired as over 10 years now and have following questions: 

1) can you still use the D27 form they sent back 9 years ago? 
2) or should I just get the D1 form from post office 
3) will I need to send new passport photo & get it verified by someone?
4) will I need to verify ID? as my passport expired in 2022.  Will this be ok? 
5) my address has changed since my last photocard and paper licence show old address.  How do I change address on the same form? 
6) Will I have to send card/paper licence? 7) will I have to send passport back to them? (As need to renew passport too soon and need it) - I can’t find birth certificate. 
8) what start date do I put for the licence?

    if your licence has expired I would have thought you might need to start from scratch and pass your driver's test again but there's nothing specific about that online that I can see.

    I think you'll need to apply via the forms at the post office as your passport must be less than 1 year out of date.  So new photos - get them taken at the post office so that they can check your application as well.

    The photocard may have expired, but your entitlement to drive won't have (unless you're now over 70). As you have the paper licence you should be able to get a photocard licence and a change of address - but you'll have to send your licence to DVLA.

    No requirement to retake a driving test unless the court specifically ordered it at the time he was banned - which would be very unusual for a routine drink driving case.

    As the ban was for more than 56 days his old license has been cancelled and he can't just update the address on it - he will need to apply for a new one using either the D1 form from the Post Office, or the D27 form that he was sent when the ban expired. Not sure whether there's a time limit on how long he can use the D27 for, but if I'm there is then the form itself should say what it is.

    As the ban was for more than 56 days.... 

    Just for information (and it makes no difference here) it is "56 days or more", not more than 56 days.
