Entries close on Wednesday, March 5.

There are five pairs of tickets available for each of the England vs Australia and Ireland vs New Zealand matches on September 6 and 7; nominees must be available for both dates.If your nominee is under 18 they will need an accompanying adult to go with them.All the names will be put into a hat and the winners announced in mid-March.
