Fit Note and UC50 form.

So I woke up panicking this morning because it dawned on me that I didn't include a copy of my fit note when I sent off my UC50 form. I thought I read that they weren't interesting in that but now I'm not sure. Should I have sent my fit note with the form and can I still send it if so? Or have I screwed up. 

Another quick question is online it states my digital fit note must include the healthcare professionals name but on my fit note the issuers name states 3. Meds (Dr) with the issuers profession being Doctor. Does this mean that it's signed? The rest of the note is fine, but I'm worrying about every little detail at the moment which isn't helping. 

Appreciate any help.

    No you do not send copies of fit notes with UC50

    You are replying on Doctors to ensure they issue a compliant fit note with everything completed correctly.  They just enter the basic details with their typed name. They don't physically sign it, as it is a digital fit note.
    The comments I post are personal opinion. Always refer to official information sources before relying on internet forums. If you have a problem with any organisation, enter into their official complaints process at the earliest opportunity, as sometimes complaints have to be started within a certain time frame.

    That's good to know I didn't need to send a copy of the fit note. 

    The fit note itself seems fine, all the details are filled in, I was just concerned by the issuer's name which seemed odd to me. My healthcare details, the reason for the fit note and everything else is on there and its in my medical records online so maybe I'm just worrying.
