I have received a text this morning
“ Your PIP review is complete. You should receive your decision letter in 2 weeks. Once you receive your letter, contact us if you have any questions. Do not call before you get the letter as we will not be able to tell you the decision. Please remember to contact us if your circumstances change.”

What is the form I should ring and ask for which tells me what they scored me on and why etc please ?

Obviously if I don’t agree with the decision I will have everything I need to ask for a mandatory reconsideration rather then wait for the letter to arrive and then ask for it and wait for that to arrive

It would be the assessment report you would need to ask for, if there is one. If the decision was made without sending your file to the health assessment providers there will not be a report written.
When you ring you can also ask what the decision was and they will tell you, even though it tells you not to ring them.0 -
Form depends on what assessment was made - and in some cases rarely there isn't even a referral for independent advice so there is no report.
So I would just call DWP PIP and ask for a copy of the assessment report... they should be helpful but getting through to an agent may take the best part of an hour.
For clarity, the independent advice/assessment report is not the decision but as we know it is usually effectively rubber stamped by DWP decision makers. You should not request a Mandatory Reconsideration until you have the decision as otherwise you are challenging something that doesn't exist or you do not have sight of.

Did you have an assessment at all? If not then it will be a paper based assessment so there may not be a report available to request.0

Took 28 mins to get through
decision was made without a review the man said and my award has been extended
