I missed an LCW group meeting and didn't explain why

As the title suggests, I logged in to my UC this morning and found out that, apparently, I had an appointment - a group LCW meeting as I'm LCW - on the 5th November (which I missed) and had until... Yesterday to explain why I didn't attend.
I did have a good reason (I had to attend a Prescribing Review with my drug and alcohol charity's doctor; it was my final warning meaning they would have stopped my prescription had I not attended) but the truth is that I was completely unaware of it until a few hours ago. I normally get a text reminder, which I didn't this time and it's apparent that I haven't been logging into my account as often as I should.
I'm freaking out quite badly as I believe I already have a warning from last year when I missed an advisors call.
The only thing on my journal is an automated message that I missed an appointment and an email from last week asking why & stating that I had until the 12th, today being the 13th.
I suffer from pretty debilitating anxiety (partially due to my on/off addiction to illicit anxiety medication. Currently I'm not only back on but also at a tolerance where seizures are a real possibility should I quit abruptly) and my life has spiraled in recent months. As I'm barely leaving the flat I'm not really eating and have missed paying some bills, resulting in a summons - which I'm in the process of dealing with. I've largely 'given up' but am still extremely worried at the prospect of a sanction and what it could mean for me.
I received an updated message at 9am, after I read about this, stating that I have to contact them to rebook and that sanctions *are* being considered. Should I state my reasons there? As weak as they are? It's in my commitments to regularly log on, and I imagine "I didn't know" would not go down well.

I left a panic stricken message that I'm embarrassed about and regret sending:
"HelloI'm hoping to rebook my missed appointment. I am in a bit of a blind panic as I logged in to my account a little earlier and have tried to contact people I know to ask for advice but nobody has got back to me. I did have a reason, though I've missed my chance to explain (I had to attend a prescribing review with my drug & alcohol support groups Doctor regarding my anxiety medication addiction - it was my final warning as I hadn't attended two previous appointments due to anxiety and not leaving the flat very often, and was in risk of having my prescription cut off. I believe I can verify this). I also have issues with my email app, as it's full of junk and doesn't always work and didn't get any text alerts. I'm aware that this doesn't excuse or explain so I can only apologise and state that I take this very seriously. I've lapsed in other areas in recent months and I'm currently panicking, so sorry if I'm being unclear. I'll be available whenever you want to see me. Once again, I apologise." Sorry for the poor formatting! They've rebooked my appt but sanctions are being considered (and likely given, I'm assuming). Does anybody know how bad it will be? It will be my first

You should be able to look back in your journal and find the original notification of your group meeting (group session). Might be an idea to find that original notification and post in here what that notification said.
Regarding sanctions: the basic scenario is that when you miss an appointment/activity and you are sanctioned, the sanction lasts for as long as it took for you to request that appointment/activity be rebooked (and you actually attend or complete the rebooked action/appointment) plus, a fixed period of 7-days. Subsequent sanctions escalate to a fixed period of 14 or 28 days dependent on when the subsequent failure ocurred.2 -

I was informed today that they've decided not to sanction me on this occasion, have rebooked another group meeting (which I've marked in my phones calendar) and have made it clear that I'm on very thin ice.
I apologise if this thread was a little dark. I over shared, due to being in a bit of a state, and feel somewhat embarrassed.
