    SSP pays for 28 weeks.

When they have to notify you depends on if it was expected that you'd still be ill or not. They should provide you with a SSP1 form either within 7 days if it ends unexpectedly or before end of week 23 if you're not anticipated to be back to work by the end of week 28

Yes, they should have informed you before SSP ended.  They should have issued you with form SSP1 informing you of the date they will no longer be paying SSP in order for you to claim UC or ESA.

You need to claim ESP details here
    https://www.gov.uk/employment-support-allowance/how-to-claim
This should be backdated to when SSP finished.

You say "My employer just  notified me....."  Have they provided an SSP1 form and returned any fit notes they have that go beyond the end of SSP.  If not request these from them but I would not wait for these before getting the claim in for ESP

EDIT  As poppy says should read ESA not ESP    Whoops

To be clear it's ESA they need to claim and not ESP. Just incase the OP gets confused between ESP and the link you posted.
