Power of Attorney for Health Witness Question

Hello everyone, I am hoping someone with experience of filling in the Power of Attorney for Health form can help. My question relates to the donors about witnesses.

Section 5 (Life Sustaining Treatment) and Section 9 (Signature Donor) are both signed by the Donor at the same time, does anyone know if the witness to both can be the same person?

All it says on the form is the witness must not be an attorney or replacement attorney appointed under this LPA and must be aged 18 and over. So I get that, but just wondering about witness/witnesses for these two sections. Thanks in advance.

Yes they can (as long as they are a legitimate witness). We used the certificate provider on ours.

Many thanks membu60. Thank you for confirming. Yes they are legitimate . The witnesses will be a cousin and second cousin.
