Hc3 certificate help with cost

I've recently received a hc3 certificate and in the box it says £49.26 per course of treatment or prescribed item, 
I'm looking at to seeing if I can get my broken crown fixed, if I can will I have to have the above amount towards the total cost of the treatment ? Its not clearly explained on the letter and I've never had one before

    In order to get the HC3 certificate, did you receive it because of low income?

    I've got one too, and the maximum I have to pay towards any dental treatment would be if it's over approx. £169. Anything under and I'd have to finance it.

    I thought the figure would have been much lower. For example I have to pay for a small filling which will cost about £73.

    It means that you pay the first £49.26 per course of dental treatment, and the NHS funds the rest.
