UC50 Telephone Assessment

Hi everyone, I have upcoming telephone assessment for WCA however i am clueless as to what does it involve and how to prepare for it. Tried looking forum but could not find related info.
Also since she sending the form, my gp has prescribed with other medication along with other which I ma already taking ( suffering chronic piano for more than 10 years body pain, neck back , chest and being diagnosed under process by a rheumatologist , gp recon i might have fibromyalgia) .
I have also just found to have anxiety disorder and have been put on one to one NHS CBT therapy. I don't know how could I inform them about my debilitating condition or should I mention them during telephone assessment. I have no idea where to look for help?

Yes fully explain the new conditions at the assessment, concentrate on how it makes doing the UC50 activities difficult.

ESA50 is the same as UC50.

They assessor is going to ask you a series of questions about your health and how this impacts on your ability to perform everyday tasks.
