Car insurance payments don't go through

Hello. Any help for this query much appreciated. I buy all sorts of things online using various cards no problem, holidays, clothes, whatever. But when I buy car insurance, I regularly get 'payment declined' or 'we can't insure you' right at the end when I have put my card details in and done the verification. Literally right at the end, when it should say 'Thanks for buying', instead it says "sorry, we can't sell this to you". Happened like a dozen times over the years, different cards, different cars. But only ever car insurance - I can buy anything else no problem. I have a gleaming spotless credit record so no problem there. It can't be anything on the actual insurance quote, because if there was a problem there they wouldn't give me a price.
The only factor in common that I can think of is that I use Quidco each year. But surely that can't be a problem? any thoughts, please?

I found out what it was - a previous insurance company had incorrectly added a past incident to my record instead of someone else's.
