CAP A5C FORM

Please can anyone help me complete this form. The probate has asked to complete this form and return it.
my late father only left a house in his will to my mother since my mother has passed away (left no will) and then to me. Applied for probate after my mother's death. The probate received a clause that I'm the executor but with my mother's interest in the property. Land registry will not transfer into my name because on the probate its shows my mothers interest in the property. I have applied to the probate to have this clause removed. after 6 months I have asked to send in a completed CAP A5C form.
The form seems very straightforward because only a house is an asset. I don't have a solicitor so do I put my name and address at the top where it says solicitors? and secondly where its says The (a) and registry what goes in there? is that all I need to fill in beside the asset in the next page. I don't need to affirm the deaths only required if death occurred before 1975. Ah ok half way down the page where it says 1/We name and address including postcode will that be my details?

Hi szofarHow did you get on with this. Was a tricky process? I have a similar position with my mum and dad's estate and am wondering if I need a solicitor to do this...Cheers0
