Tax code issue

Hi, hope everyone is well. Just wondering whether there is anyone who may be able to advise me..
Eighteen months ago,I was made aware by my employer of an issue regards my tax code.
Their third party accountant had made an error stating that in addition to my normal PAYE pay, I'd been paid a considerable amount from the holdings company that runs for the directors. Whilst I would have loved to receive this, I didn't!
This resulted in my tax code being changed, due to a perceived under payment of £500. This then lead to tax credits demanding £1200 pounds to be returned to them.
Employer had stated that the process was in hand with the accountants, with a lead time of two years approx.
I spoke to HMRC yesterday to get an update as this was eighteen months down the line, and it's causing extra strain with two small kids. I was put through to a senior member there who told me they have nothing on their system regards this issue at all. He also seemed of the opinion that this matey can't be handled by the accountant and had to be dealt with by the employers.
Would anyone more knowledgeable than I be able to give me any insight as to which of the two takes on it is correct?
And tbh how to proceed from here for a resolution...

The payroll information about your income sent to HMRC needs to be corrected.
That would be your employers responsibility, but it may be that the accountant does this on behalf of the employer.
