CASCO insurance for rental car

Hi, we are trying to rent a car in Albania .  The cost does not include CASCO insurance, which as far as I can glean, is what you need to have fully comp insurance, the renters only provide 3rd party.

Does anyone know if we can arrange this insurance as a stand alone, in the same way you would for excess insurance, or are we forced to buy that at the desk when we pick up the car?

Some excess policies you can add LDW/CDW to world wide cover... tends to be common for the US/Canada but certainly some do world wide. 

CASCO actually goes further than Comprehensive insurance does as it covers injuries to the insured
